About this template

Here is a Quiz Template that comes with three sections, first as to collect submitter name and email address, the second part where the questions are shown and the last part where the submitters see their score and if the score is not 100% they are allowed to decide whether they want to retake the quiz or submit. Once submitted the users can drop their comments and feedback. We've prepared the Quiz to measure users' knowledge about JotForm. You can customize the template, change the content, add your visual and informative material, provide dropdown, multi-select, image select options to your users, add/remove fields, change the colors, fonts, and background and either embed it to your website or use it as a standalone form.
