Employee Discipline Form
Employee Discipline Form is a document that is used when an employee violates a policy of the company intentionally. It is expected as an employee that he/she will follow the established rules and policies of the company where he/she is working for. When he/she got hired by a company, the employee will normally sign a form indicating that if you don't follow the rules of the organization, a process will be followed on how will the employee be reprimanded. It could be a verbal warning, written warning, or even suspension depending on the gravity of the offense.
This Employee Discipline Form template contains form fields that ask for the employee's name, ID, contact details, position, and department. This form also asks what is the reason for the disciplinary action. It could be related to attendance, disobedience, safety, respect, quality of work, and many more.
