The attached document is of the original questionnaire form used to create the original form, Background Profile. I worked at Franchise Building Expert for about 10 yrs and now I work for myself and do not have access to that email. I was just informed that I still had "FBE" logos on my form and I saw my email was still the old one. When I went to change it I needed to verify through that email which doesnt exist any longer.

I am on an American Airlines flight to Italy. I was loading up a Jot form document for a Grant I was applying for. At the very end, I was trying to attach an excel file and lost EVERYTHING. All my work that had been carefully loaded over several hours was lost.
I missed the deadline for the grant minutes later.
if you can find my work it was grant for US Rowing's United We Row program.

I have an interesting scenario. A former employee send an Email address change request last year prior to his departure. Unfortunately, it ended up in my spam box and I completely forgot about it until now, when we need to access our form. I still have the transfer email, but we no longer have access to the former employee's email. Can someone resend me the transfer email or is there another way that we can make sure my email replaces the former employee?
