Now, coming back to your question, yes, it is possible to automatically integrate a Surname and First Name based on Agency data in your form using both Conditions and Prefill Data.

With Conditional Logic, it would work well if you have a predefined list of agencies and their corresponding names. You can set up conditions to automatically populate fields based on the selected Agency. Here's an example based on your form with a dropdown field for Agency, but you can adjust the condition as needed. Let me show you how:

1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
2. Click on Conditions on the left of the page.
3. Now, click on Add a New Condition and then click on Update/Calculate Field.
4. Set up the conditions as you see below and click on Save.

With Jotform Prefill Feature, you can autopopulate the Surname and First Name. You can choose which Prefill method suites you best, I used the Manual Prefill Feature as an example here. This allows you to pre-populate fields with data before the form is shared. Here's how:

1. In Form Builder, go to the Publish tab.
2. From the left side menu, click on Prefill.
3. Click on Manual Prefill. 
4. Select the Add a New Prefill button. This should open the form where all your data are that you can use for further prefilling.
5. Enter all the values for the fields that you wish to prepopulate.
6. Finally, click on the Create button at the upper right side of the page. 

You can also prefill your form with data using the ff:

• Forms
• Tables
• Salesforce
• HubSpot
• Airtable
• SSO
• Pipedrive
• monday.com

Additionally, the Spreadsheet to Form Widget is another option; it allows you to prefill a form with data from an XLS, XLSX, or CSV file. Let me show you how to use it:

1. Create a spreadsheet with your data structured like you see below. Make sure that the column names in the spreadsheet match the field names on the form, and include a unique column, like an ID, so the data can be distinguished.

2. In Form Builder, click on the Add Form Element menu on the left side of the page.

3. Click on the Widgets tab, search for the Spreadsheet to Form widget and add it to your form.

4. In the menu on the right side of the page, upload your data file. (You can simply drag & drop your excel file to the Upload File area.) 

5. Then, click on the Downward Arrow icon in the Access Code Column option and select the unique column.

6. You can also customize texts used in the widget. After you complete everything, click on the Update Widget button at the bottom right of the menu.

Now, when people fill out your form and enter the unique code from the spreadsheet, the corresponding fields will automatically populate when they click Auto Complete Fields. Check out my screencast below to see how it works:

I see. Jotform treats the Full Name element as a single field, making it difficult for conditional logic to update the fields separately. As a workaround, you can use two Short Text fields instead to populate these fields based on the selected option in the Agency field. Here's how:

1. In Form Builder, click on the Add Element button on the left side of the page.
2. Under the Basic tab, click on the Short Text element twice to add two Short Text elements on your form. Label these as Prénom and Nom de famille.
3. You can shrink these fields so they could be in one row by clicking on the Gear icon, then on the menu on the right, toggle ON the Shrink section to make field smaller.
4. Next, in the orange navigation bar at the top of the page, click on Settings.
5. Click on Conditions on the left of the page.
6. Now, click on Add a New Condition and then click on Update/Calculate Field.
7. Set up the conditions as you see below and click on Save.

Unfortunately, it is not possible to pre-fill or set the First name and Last name for the Full name element using a condition depending on the submitter's answer. While the feature you’re looking for isn’t currently available, rest assured we’re always working to add new services and features to Jotform. I've gone ahead and escalated your request to our Developers. When or if it's implemented depends on their workload, how viable it is, and how many other users request it.
