This form is not sending both image attachments to google groups or google drive. The first image will appear for both image submissions. The 350x250 will also appear in the 1500x750 slot. But if I look in my jotform inbox, the images are correct.Also, I am unable to attach screenshots to this support ticket.

I cloned your form to see if I could replicate the issue, but everything is working as expected. Take a look at the screenshot of the result below:
Can you try setting your Clear Hidden Field Values settings to Don't clear and try submitting an test entry again? Let me show you how:- In Form Builder, click on the Settings tab in the orange navigation bar and click on the Show more options button.
- Now, scroll down to the Clear Hidden Field Values, click on the Downward Arrow icon and select the Don't clear option.

After checking your form, I found the reason why it was not sending the attachments. See the screenshot below:
The error message indicates that the integration is trying to upload files to a folder that either:
- Has been deleted,
- Has had its permissions changed, or
- Is no longer accessible due to a disconnected or reauthorized account.
To fix this, disconnect your Integration and add it back. You can remove and reintegrate your form with Google Drive in just a few simple steps. Let me first show you how to remove the Google Drive integration:
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
- Then, in the menu on the left side of the page, click on Integrations.
- Next, search for Google Drive and click on it. Then, click on the Three Dots icon on the right side of the page and select Delete Action.
- In the Confirmation window that opens, click on Yes, Delete.
You can then reintegrate your form with Google Drive. Here’s how:
1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
2. Then, in the menu on the left side of the page, click on Integrations.
3. Now, search for Google Drive and click on it.
4. Next, select an email address to use a saved Google Account, then click on Authenticate on the right. Or click on Use Another Account below the Dropdown menu to log in and allow access for a different one.

5. Set up the Google Drive settings as you want and click on Save.
