Form Builder: Hidden Text Appearing on Published Form Not Visible in Editor

in addition to my initial request I am having an issue of verbiage showing on the published form that I don’t see when I build the form an AI says they cannot see it/remoe it.
I am trying to delete the “I am ACCEPTING travel insurance and authorize the travel purchase” underneath the initial box - I do not see this line in the build app. But it displays as soon as I publish the form.

Thanks for using Jotform. I cloned your form and found the extra text. The line I am ACCEPTING travel insurance and authorize the travel purchase comes from two multiple choice fields still active in the Form Builder. Deleting those fields will remove the text from the published form. Let me show you how:

1. In Form Builder, scroll down and stop before the Signature field.
2. Select the Multiple Choice field.
3. Select the Trash icon.
