if the employee email empty and supervisor email empty
The direct manager is responsible for completing the employee's performance evaluation form. The form must be signed by the manager prior to submission

Hi Hussam,
You can easily achieve this logic using the Autoresponder Email and Conditional Logic. When the form submission has the Employee Email and Supervisor Email fields empty, then the submission would be forwarded to the Direct Manager to complete the performance evaluation form. Let me show you how:
First, let's set up the Autoresponder Email that will be forwarded to the Direct Manager when the Email fields are empty. We'll add an Edit Link for the Direct Manager to be able to edit the submission. It's easy, let's do it:
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
- In the menu on the left side of the page, click on Emails.
- Hover your mouse over your Autoresponder and click on the Pencil icon to customize it.
- Click on the Email Content window, then click on Form Fields, and select Edit Link.
- Customize the Edit Link the way you want, and when you're finished, click on Save on the bottom-right side of the Email Content window.
Now, let's add the Signature field and the Get Form Page URL widget on the form for us to be able to get the Direct Manager's signature when they edit the submission. Let's do it:
- In Form Builder, click on the Add Element button on the left side of the page.
- Next, under the Basic tab, click on the Signature element to add it to the form. Ensure that the Jotform Sign is disabled to allow submission editing.
- Click on the green Pen icon beside the Signature field and on the next window, toggle Jotform Sign Automation to OFF.
- Now, let's go back to the Build tab on the orange navigation bar.
- On the Form Elements panel on the left side, go to the Widgets tab.
- On the Search bar, search for the Get Form Page URL and click to add it to the form.
Next, we'll set up the conditions. Let me walk you through it:
- In the orange navigation bar at the top of the page, click on Settings.
- Click on Conditions on the left of the page.
- Now, click on Add a New Condition and then click on Change Email Recipient Field.
- Set up the conditions as you see below and click on Save.
- Next, click on Add a New Condition button and then select Show/Hide Field.
- Configure the condition below and click on Save.
- Then, click on the Add a New Condition button again and select Enable/Require/Mask field.
- Finally, apply the condition accordingly and click on the Save button.
This way, when the Employee and Supervisor Email fields are empty, the Direct Manager will receive the Autoresponder Email to complete and sign the evaluation form before final submission. Check out the screencast below for the result upon form submission:

- Access Conditional Logic: In your form builder, navigate to Settings > Conditions.
- Create a New Condition: Click on Add a new condition and select Skip to Page / Thank You Page. https://www.jotform.com/answers/4001956-conditional-logic-for-single-choice-field
- Set the Condition: Define the condition to check if the specific fields are empty. For example:
- If: [Field Name] Is Empty
- Then: Skip to a specific page or show a message prompting the user to complete the form.
- Assign to Another Person: If you want another person to complete the form, you can use the Assign Form feature. After setting the condition, assign the form to the intended person
