Now, coming back to your question, can you share with us the form URL in question so, we can check? Are your form fillers using the mobile device or desktop version while they are submitting the form? Is your form is embedded on the form website? If yes, can you share with us the embedded form's URL? If needed, can we run a test on your live form to see if we can replicate an issue at our end? I've cleared your form caches; your forms are now refreshed.
