¿Cómo puedo crear una tabla de doble entrada en donde en cada campo tenga una lista desplegable distinta?

Now, coming back to your question, you can use Input table, this allows you to create two dropdown fields side by side. Let me show you how:
- In Form Builder, in the orange navigation bar at the top of the page under the Build section, click on the Add Element.
- Choose Input Table. Drag and drop it on your form.
- Click on the Gear icon to open Properties. On the right side panel, click on the Fields tab.
- Choose Multi-Type Column in the Input Type.
- Scroll down a bit, and click on the Downward Arrow icon in the Column Types section.
- Choose Dropdown, the last option. That's it.
