Como hacer un formularios de asistencia dinamico

tengo un formulario con codigos unicos son muchos pero yo quiero que cada vez que un codigo sea registrado el mismo codigo varias veces no me cree un registro nuevo si no que se alimente el mismo registro. como la asitencia si pedrito asistio a clases se le pone un chulito por fechas. mas o menos algo asi.
