Creador de firmas

Buenos días,

Hemos creado un documento de firma e insertado en un contenido Scorm.

Una vez el usuario ha firmado el documento, le aparece un formulario de jotform instando a crear un documento de firma propio.

¿Cómo podemos quitar este formulario final?
