Hi TU_hipotek,
Thanks for reaching out to Jotform Support. Our Spanish Support agents are busy helping other Jotform users right now, so I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in Spanish, let us know and we can have them do that. But, it might take a while until they're available again.
Now, coming back to your question, when I check, I found two Smart PDF Forms that has Banco Caja Social which are Firma Banco Caja Social and Formulario Banco Caja Social and both have vertical Original PDF's and already been mapped. Can you confirm if you still need further assistance? If so, can you share with us the URL of the Smart PDF form you're working on?
Once we hear back from you, we'll be able to move forward with a solution.

Hi TU_hipotek,
I see the last page you're referring to. Unfortunately, it's currently not possible in Jotform to turn that last page 180 degree, to make it display vertically. You will have to update the document copy you have on your computer, customize the page using third party PDF editor like Adobe Acrobat. Then, once you've updated the PDF, you can re-upload it to your form. Let me walk you through how to re-upload a PDF to the Smart PDF Form.
1. In Form Builder, in the blue navigation bar, click on the Upload tab.
2. Click on the Update PDF button, and choose Re-upload PDF in the modal pop up.
3. Now, click on the Upload Document button, and choose the new updated PDF.
Give it a try and let us know if you need any more help.
