Thanks for using Jotform. I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:
I've cleared your form caches just to make sure it isn't causing any issues to your form. You can clear your browser caches and cookies, log out of your Jotform account and log back in, use incognito mode or private browsing, use different devices or browsers, connect to different networks, disable your VPN or proxy server, or temporarily disable your firewall to see if the issue has been resolved.
If the same thing happens, can you confirm that the issue is happening on your Damage Claim Form? If not, then can you share with us the link to your form so we can take a look for you? You can easily find your form's URL in Form Builder. Let me show you how to do it:
- In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
- In the Quick Share tab, click on Copy Link in the Share With Link section.
