Thanks for reaching out to Jotform Support. Can you try embedding your form using the iFrame method? Embedding your form on a webpage is really easy. Let me show you how:
- In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
- In the menu on the left side of the page, click on Embed.
- Now, select the iFrame method, and click on Copy Code.
- Paste your code into your webpage's embed area, and that’s it.

I was indeed able to embed the form, however, it now does not include the Submit or Clear Form CTA. Do you know what would be causing those to no longer be included?

I cloned and checked the Submit button in your form, and found that it's currently tied to a condition that's supposed to show it. It seems that some changes have been made to the form, where some fields were removed, and those same fields are part of the conditions linked to the Submit button. As a result, the condition is no longer working as expected, which is why the Submit button is currently hidden.
You can disable the conditions linked to the Submit button so it shows as expected again. Let me show you how:
1. In Form Builder, select the Submit button, and click on the Conditional Branch icon on its right.
2. In Conditions page, select all conditions linked to the Submit button, and click on Disable.

We’re not able to see the embedded form on your website because it requires registration or login. Can you provide test credentials so we can check the actual embedded form? Additionally, I found the form named Damage Claim Form. Based on the condition you set, you want to show multiple fields, including the Submit button, when users select how many SKUs were damaged?
If that’s correct, you may need to slightly adjust the Show/Hide Field conditions on your form because showing and hiding multiple fields at the same time can often cause conflicts. First, you need to disable your current conditions. You can follow the step-by-step guide my colleague shared above. After that, we need to create new Show/Hide Field conditions. Let me show you how:
1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
2. Next, click on Conditions on the left of the page.
3. Now, click on Add Condition, and then select Show/Hide Field.
4. Then, set the condition like you see below, and then click on Save.
5. Next, set another condition like you see below, and then click on Save.
Keep in mind that you’ll need to create multiple fields for each option in the field named How many different SKU’s were damaged? Check out my screencast below to see my results:
You can check my cloned form using this link. I only created 5 conditions for the 5 quantities in your fields. Since you have 30, you’ll also need to create 30 conditions. You can find more info in our guide on How to Clone an Existing Form From a URL.
Regarding your issue with the File Upload fields in your form. I’ve gone ahead and moved your other question to a separate thread to keep things tidy, and someone from our Support Team will help you out with it here.
