Here’s how to update your custom URL slug:
1. Go to your Workspace page and click on your Avatar/Profile Image in the top-right corner.
2. Select Settings, then click on Account in the left menu.
3. Click Edit in the Custom URL Slug section, enter the new slug, and then click Save.
4. Confirm the change by clicking Yes, Continue in the confirmation window.
Note: Changing the slug updates your custom URLs, and the old custom URL will no longer be valid. The original form URL still works regardless of slug changes.
If you want to fully customize your URL, that is available with Jotform Enterprise.

Please check whether the custom URL slug was saved in Account Settings and then try logging out and back in to refresh the account view.

Take note that the user's username will be added in the custom form URL, and removing or changing it isn't possible. If you want your form URL slug to change from https://form.jotform.com/LTIMindtree to https://form.jotform.com/LTM, you'll need to update the username of your Jotform account from LTIMindtree to LTM. Changing your Jotform username is easy to do. Let me help you with that:
1. On your Account Settings, click on Edit in the Username section.

2. Enter your new username and click on Save. That's it.
If HIPAA compliance features are enabled in your account, changing usernames is currently not possible. Additionally, changing your username will invalidate All Custom Form URLs that include your old username.
