Form Display Issue: Unwanted Bracket with Object Showing Next to Checkbox

Can you tell me what this bracket with object is on the form? I did not type this and I would like that removed to only show the option to check the box. SEE SCREEN SHOT ATTACHED!

Hi Kehaulani,

Thanks for reaching out to Jotform Support. You can easily remove the option added in the Single Choice element. Let me show you using a cloned form:

1. In Form Builder, select the Single Choice element at the bottom of the form.
2. Then click on the Gear icon on the right side.
3. In the Single Choice Properties on the right side, go to the Options tab.
4. Finally, remove the option.
   
   

I have a suggestion that you can check out. You can try using the Terms & Conditions widget. Here's a demo you can follow:

1. In Form Builder, click on the Add Element menu.
2. Under the Form Elements, go to the Widgets tab. Look for the Terms & Conditions widget, then drag and drop it in whichever part of the form page you prefer.
3. Next is to select the Terms & Conditions widget and click on the Wand Icon on the right side.
4. In the Widget Settings on the right side, add the text inside the box below the Label Text. Finally, click on the Update button.

Emergency. Please help. Why is the check box with the privacy and terms and condition question showing up on the 1st question. Please help me remove ..... I dont know how that happened. SEE screenshot. That question should be at the bottom of the survey.

Hi, its still there. Please help me fix this asap. The NASCAR team is testing it and this doesn't look good. Its still showing up. I clicked on your link and same thing popped up. see NEW screenshot

You're seems referring to the Clone of NASCAR Fandom Attendee Survey Draft form. Go to this link to open it in Form Builder. Click on the field to select, and then click on the trash icon on its right as it shows in the screenshot below:

Wait. I hit trash, but will this still how up at the bottom of my survey? This is a legal opt in/out question that has to be on the form. I just need to not have it show up at the top of the form on the first question. Please confirm if you want me to delete this question, but it will still show up at bottom of form. See screen shot of how it is showing up on top of form.

The reason why the Terms & Conditions widget is showing at the top of the form is because of this condition you created below:

Currently, this condition is incomplete. To fix this, you either need to disable or delete this condition so the Terms & Conditions widget will be correctly placed at the bottom of the form. Here's how:

1. In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.
2. Click on Conditions on the left side of the screen.
3. Next, hover your mouse over the Gear icon on Condition #1. 
4. Lastly, select Delete or Disable. 

Here's the result:

If I delete that option, will this impact the other questions in that same condition. Your team told me to set it up this way based on the conditions required for question #1. Were you able to review last night notes and changes we made for the conditions?

See when I disabled the condition. It now shows all the questions again. We had just fixed that last night.

Question 1 has 2 workflows / paths.
