Unable to Add form in Shopify Store

    Hi there,

    I just tried to do this but it doesn't work..

    Hi CareMate Pty Ltd,

    After testing, I was able to successfully embed the form on my Shopify Contact page. Take a look at the screenshot below to see my results:
