tombuehlAsked on August 3, 2025 at 4:29 PMIs it possible to have multiple file fields within the same form? Example. While 90% of our tickets will only be for one team... a few will involve multiple teams. What I'd like to do [if possible] is have a File Upload field in one section, then another, and then another on top of that [since each team is looking for something different].
If not, no worries. Just curious if it's possible. If so... how, because I don't see the ability to map file fields.

Hi Thomas,
Thanks for reaching out to Jotform Support. If I understand correctly, you want to insert multiple File Upload fields on your ClickUp integration to be able to add multiple attachments to the ClickUp record. Is that right? If so, yes, you can add multiple File Upload widgets on the Attachment section of the ClickUp integration. Check out the screenshot below:
If that’s not exactly what you’re looking for, can you explain a bit more so I can help you better?
Once we hear back from you, we'll be able to help you with this.

Thanks, Shaina. Appreciate the fast response. Yeah, I eventually found that. Based on your screenshot, it looks like you can include the multiple file upload fields. Curious, though... how do those come across in ClickUP? Example. We have one main "list" that [based on automated rules] will route the form requests to one or multiple lists. We're a Marketing department, so we have a list for Creative... Digital... Comms... Social... and Web. 90% of the time, the people who fill out the form only one one team (example. They want Creative to create a graphic or video), but SOMETIMES... they need help from all of us (Creative to create the graphic, Social to post to LinkedIn, and maybe Web to post to our website). It's likely that the person filling out the form will only have attachments relative to one team (so a generic "Attachments" field is probably fine), but... as we were building out our form... some of those teams (Web, for instance) wanted to have specific file uploads for specific issues (bugs, images for a web page, a contact list for Digital, etc.). If I follow your example (above) and choose all those file upload fields in Jotform... how will they be separated in ClickUp (if they are). As an example, if I have a "Screenshot" field for bug issues (in the Web section of the form), how will the Screenshot files show up in ClickUp (since you can't map)?
I'll keep playing around with it. I went ahead and deleted the separate upload fields, but I'll try creating another one and test that out (to see what ends up being displayed in ClickUp). Sorry for the novel. :)

Hi Thomas,
You can definitely include multiple File Upload fields in your form and map them in the ClickUp integration; but, it's important to note that all uploaded files from those fields will appear collectively under the Attachments section of the ClickUp task. Unfortunately, ClickUp doesn’t retain the labels or field names from Jotform, so there’s no built-in way to distinguish which file came from which upload field once they’re in ClickUp.
That said, you can try to label each File Upload field clearly in your form like Creative Team Uploads, "Web Screenshots, or Social Media Assets, so the user knows which files to upload where. Additionally, you might consider adding a short text field next to each upload field where users can describe the purpose of each file, this information can be mapped to ClickUp fields like the task description or custom fields, giving your team the context they need to identify which files are for which team.
Let us know if you have any other questions.
