Hi cc00,
Thanks for reaching out to Jotform Support. Can you confirm if the new form that you're referring to is your Live Illustration Additional Options form? To my understanding, if the Custom Paper Template Design is selected from the Product List field of your Live Illustration Event Booking Form, you want fields from the Live Illustration Additional Options form to be shown or hidden. Is that right? Can you be more specific on what fields will be shown and what fields will be hidden?
After we hear back from you, we'll be able to move forward with this.

yes, I will create a field to show for the custom templates, they will add the text they want on the custom templates and upload any images or describe what kind of background they'd like...or I can send another form for this is fine

Since it’s not possible to use conditions to check if a specific product is selected within the Product List element, instead of removing the entire Product List and recreating it using native fields like Single Choice, Multiple Choice, or Dropdown, I suggest removing the Custom Template Paper Design from the Product List. You can then use a Terms and Conditions widget to ask users if they’d like to include it, especially since it’s a free item. Let me show you how:
1. In Form Builder, click the Product List element, select the Custom Template Paper Design product, and click the Trash Icon on its right.
2. In Form Builder, click on the Add Element menu on the left side of the page.
3. Under the Widgets tab, drag and drop the Terms and Conditions widget into your form, below the Product List element.
4. Label the Terms and Conditions widget as desired.
5. In Form Builder, click on the Add Form Element menu and under Basic tab, add Short Text field below the Terms and Conditions widget.
6. Label the Short Text field the way you want it.
7. In Form Builder, click on the Add Form Element menu and under the Basic tab, add a File Upload field below the Short Text field.
8. Label the File Upload field the way you want it.
9. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
10. Click on Conditions on the left of the page.
11. Now, click on Add a New Condition and then click on Show/Hide Field.
12. Set up the condition as you see below, and click Save.
Lastly, set the Terms and Conditions widget to not required, as users might choose not to include Custom Template Paper Design.
13. In Form Builder, select the Terms and Conditions widget, and click the Gear icon on its right.
14. In the menu that opens on the right, under the General Tab, set the Required section to No.
That's it. Give it a try and let us know how it goes.

Hi cc00,
Since you're using Multiple Choice fields, you can set the Entry Limit to a maximum of 1. That way, users can only select one option. Let me show you how:
1. Open your form in Form Builder, then select the Multiple Choice field.
2. Next, click on the Gear icon to open Multiple Choice Properties.
3. In the menu that opens on the right, click on the Surveying tab, then toggle Entry Limits option to On position.
4. Set the Maximum to only 1, and you're done.

I think I want it to operate like a radio button

Hi cc00,
Right now, that’s the closest way to make it work like a radio button or a Single Choice field. If you’d like, you can just replace it with a Single Choice field since it’s already built for that purpose.
