    I want to make it so that when this product is selected at checkout submission then another form will be sent automatically they can fill out for additional questions e.g what they want on their paper design

    and to show or hide fields in the new form if this checkbox product is checked or not

    and to know which form this new form was the additional submission of without adding their name again

    As for your original question, it's easy to set the Email notifications or Autoresponse email based on the answers of people who fill out your form. Let me show you how:

    1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.

    2. In the menu on the left side of the page, click on Conditions, and then select Change Email Recipient. (If you already have a condition set, first click on Add Condition, then select Change Email Recipient.) if product is selected on form submission, send an auto respond email so that another form can be filled Image 1 Screenshot 40
