...need some help in understanding how to get my form either to my website or my FB page where I upload it and receive completed submissions. I've seen other puppy breeders use Jotform but I need some guidance on how to actually get my Puppy Questionnaire working and then get submissions back to me.

To get your Puppy Questionnaire form working on your website or Facebook page and receive submissions, here’s how to do it:
- Embed on your website: Go to your form's Publish tab, select Embed, then choose the iFrame code. Copy this code and paste it into the HTML of your website where you want the form to appear.
- Add to your Facebook page: Use the Platform - Facebook option in the Publish tab to get the embed code specifically for Facebook. You can add your form as a tab on your Facebook page using this method.
- Receive submissions: All submissions will be collected in your Jotform account. You can view them under My Forms > Submissions or set up email notifications to get alerts when someone submits the form.
This will allow you to collect completed Puppy Questionnaires directly from your website or Facebook page.

Hi Kathy,
You can easily embed your form on your website using the Iframe code. Embedding your form on a webpage is really easy. Let me show you how:
- In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
- In the menu on the left side of the page, click on Embed.
- Now, select the embed option you want to use, and click on Copy Code. (I used the iFrame method only to show in this case.)
- Paste your code into your webpage's embed area, and that’s it.
- In Form Builder, click on Publish on the orange navigation bar.
- Then, click on Platforms on the left panel.
- Look for Facebook and complete the setup.
