Hi Tony,
Thanks for reaching out to Jotform Support. I checked the account linked to the app you shared, and it's currently active, but based on the screenshot you shared, the form that you are trying to access within the app has a password protection.
If you can't access your account, you can reset your password. Here's how:
1. Go to the Jotform Main page and click on Login at the top-right corner of the page.
2. In the window that opens, click on Forgot Password?.
3. Enter your email address, and then click on Send Reset Instructions.
4. You'll receive an email with instructions on how to reset your password. Open the email and click on the Reset Password button.
5. Enter your new password, enter it again to verify it, and then click on Create a New Password.
If you forgot your form password, you can also reset it in Form Builder. Here's how:
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
- Under Form Settings, click on Show More Options and click on Reset Password under Password Protection section.
