    Another question: How do i set for respondent to enter their % for each components as below?

    In a typical month, what percentage of your time spent with PLwD and/or their caregivers is dedicated to the following broad areas? (Total must be 100%)

    Direct assessment of PLwD's communication skills: ______%

    Direct impairment-based interventions (e.g., word retrieval drills for PLwD): ______%

    Caregiver communication training/education/counseling: ______%

    Addressing swallowing difficulties (dysphagia): ______%

    Liaison/collaboration with other professionals/family meetings: ______%

    Other (please specify): ______% Total: 100%

    Hi puspa1301,

    To collect percentage values and ensure they total exactly 100%, I suggest using Number fields for each input component, a Form Calculation widget to sum the values, and conditional logic to either display a warning message or disable the submit button if the total does not equal 100%.

    1. In Form Builder, click on the Add Element menu on the left side of the page.

    2. Under the Basic tab, scroll down to Number element and add six to your form field and label each of them with % at the end.

    3. Click on the Add Element menu again, click on the Widgets tab, then search for the Form Calculation widget and add it to your form field.

    4. Click on the Wand icon beside the Form Calculation widget then click on the Add Field button.

    5. Now create the formula for using the Plus icon between fields and % at the end. You can follow the screencast below and click on the Save button once done.

    6. Click on the Add Element menu, scroll down to the Paragraph element, and add it to your form field.

    7. Next, click on the Pencil icon beside the Paragraph field, create your custom warning message, and click on the Save button beside the field once done.

    8. In the orange navigation bar on the top of the page, click on Settings and click on Conditions on the left side of the page.

    9. Click on Add Condition and select Show/Hide Field conditional logic.

    10. Now create the conditional logic like the screenshot below and click on the Save button once done.
