Hi there, can i ask if i intend to put a screening question at number 10: so for those who answered NO i want them to submit the questionnaire without proceed to other sections, how do i create this option?

Dementia caseload: In the last 12 months have you provided assessment or intervention to at least ONE (1) adult living with dementia-related communication impairment?

    Yes (Please continue the survey)

No (Thank you for your time. Please submit your responses now. Your answers to Section 1 will help us understand the demographics of SLTs reached by this survey.)

Another question: How do i set for respondent to enter their % for each components as below?

In a typical month, what percentage of your time spent with PLwD and/or their caregivers is dedicated to the following broad areas? (Total must be 100%)

Direct assessment of PLwD's communication skills: ______%

Direct impairment-based interventions (e.g., word retrieval drills for PLwD): ______%

    Caregiver communication training/education/counseling: ______%

    Addressing swallowing difficulties (dysphagia): ______%

    Hi puspa1301,

Thanks for reaching out to Jotform Support. I reviewed your form and noticed that you’ve tried to set up a Skip To/Hide Page condition that redirects users to the page with the submit button. As another method, you can also place a submit button directly on page one by using a divider and applying Show/Hide Field conditional logic. This way, the form won’t redirect to the last page, and the form filler can submit the form immediately when No is selected for Question #10. Let me show you how to do it:

    1. In Form Builder, click on the Add Element menu on the left side of the page.

    2. Under the Basic tab, scroll down to the Divider element, then add it above the Next button on page one, and add another Submit button above the Divider field.

    3. In the orange navigation bar on the top of the page, click on Settings and click on Conditions on the left side menu.

    4. Click on Add Condition and select Show/Hide Field condition.

    5. Now create the Show/Hide Field conditional logic like the screenshot below and click on the Save button.

As per your other question, I moved it to another thread to avoid confusion. You can check it out at the link below:
