Hi Timothy,
I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:
Can you try to create a new PDF document? Let's do it:
- In the blue navigation bar at the top, click on the New PDF button.
- In the windows that opens, choose Use Default Layout and that’s it.
Give it a try and let us know how it goes.
