Unable to submit form

This did work but now the form will not allow users to submit the
form.

Can you tell us more about what's happening in the form? When I checked the form, and I see that the submit button is moved to the middle part of the form:

Can you try moving the other one to the end of the form so it will allow them to submit the form? If that's not exactly what you're referring to, you can provide us with more information so we can better understand what's happening.
