Hi scfallfestival1,
I understand that you're concerned about a form you created not showing up in your account. After checking your account using our system, I cannot find any form created in it. You might've created a different account associated with a different email address considering that you had trouble logging in. Can you share with us a different email address that was potentially used when you created the form? Or, if you have the form URL, I can also use that to track the account.
