How can I get customer's name, email. phone number, and email address

Hello Jotform Team,

I'm planning to conduct a survey to gather our leads' preferences, and I would like to capture their name and contact information, such as email and phone number. Do you have a widget that allows for this?

Additionally, is it possible to change the "Submit" button text to something like "Show Me The Perfect Neighborhood"?

Thanks for reaching out to Jotform Support. You can use the Full name element to capture the Name, the Phone number field for the phone number and the Email field for the email address. Let me show you how:

1. Now, in Form Builder, click on the Add Form Element menu on the left side of the page to see the complete list of elements and widgets.
2. Under the Basic, Payments, or Widgets tabs, scroll down the list, and then drag and drop the fields to the form. 

As for changing the wording on the Submit button, this is possible on the button Properties, Let me show you how:

1. Scroll down to find the Submit button and click on the Gear icon to view Properties.

2. Edit the text for your Submit button.
