how to fill out a form in google docs

Hi, I need to find out a way to fill out forms in google docs. We're providing inspection services to those who just purchased a new house. What we're basically doing is to assess the current condition of a property and make our suggestions to new owner. I need a form or a pdf for our employees doing the inspection on the site and supervisors who are responsible for the contracts. But I'm not sure how I can create a from for this purpose.
