Hello
I am trying to capture the following on a form and have the values transferred to a PDF.
I have tried configurable list but that data cannot be transferred to the PDF. I have tried an input format but I cannot figure out how to get it to create a checkbox on the appropriate choice.
If I use a multiple choice box, I have no way to enter the value.
Can you help with this?

Hi Dora,
Thanks for reaching out to Jotform Support. You can actually pass the selected values from your form to your PDF using the Input Table field. Let me show you how:
1. In Form Builder, click the Add a Form Element then add an Input Table, and enter the options the user can fill in.
2. Click the Input Table Properties, then go to Fields. Select Multiple type columns.
3. Enter the rows and columns of your table. In the Column Types, select Checkbox and Textbox.
4. Map the fields on your table to the PDF document. Do this on all the fields.
That's it. You can now fill the PDF based on the table you set up. Here is the result:
