Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named 39 Car Rental Kuching Inspection that was flagged as offering a fake service. We understand this might not be the case with your form, but we had to act according to the general rule to be in compliance with our Terms of Use.

For us to reactivate your account, we’ll need your permission to delete the offending form including all collected data in the form. You can create the form again after changing your account email address to a corporate email.

It is for my Car Rental inspection use only. what is fake service? all data i fill in myself, only take photo is by other.

Fake services mean that the form is offering suspicious services coming from a Free account with non-corporate emails, which is automatically tagged by our Phishing system. Kindly provide us with the following information if you believe that your account was suspended by error:

• The URL where your form is used.
• Your website or proof of affiliation to a legitimate institution.
• Change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

Send these to reviewers@jotform.com with thread #18732911 as your subject, and we’ll let you know about the status of your account after we’ve done a thorough review of your proofs.
