Thanks for reaching out to Jotform Support. I'm sorry if you're having difficulties with this. I've checked your account, and it seems that you've been auto-suspended by our system due to this form having the word Impersonate in a certain field, which is against our Terms of Use.
I’ve gone ahead and deleted the offending fields and reactivated your account. You should be able to log in now. We recommend using another term for collecting this information, as it will be flagged by our anti-phishing system.
