How to Create a screener form?

How would I be able to create a screener that a person can fill out and get an immediate response via email based on the scores?

Thanks for reaching out to Jotform Support. Sure, it's easy to set an Autoresponder email based on the answers from form submitters. I'll show you how:

1. In Form Builder, in the orange navigation bar at the top of the page, click on the Settings.
2. In the menu on the left side of the page, click on Conditions.
3. In the list that pops up, click on Change E-mail Recipient and set a logical condition like the ones below:
4. Now, click on Save, and you're done.
