Order of questions on form changes after submission

On one of my forms the first two questions are being kicked to the bottom of the page when I print the forms. This question is first for a reason, how can I fix this?

May I know, after filling out the first two fields, it sends you to the bottom of the page? However, I have gone ahead and cleared your form cache. It is a good practice to delete the cache, so data are refreshed to avoid this type of issue in the future. You can do it easily. Let me show you how:

1. Hover on your Avatar/Profile picture.
2. Click on Settings.

3. Scroll down and click on Clear cache option.

Also, please try to clear your browser cache. Can you try it again and see how it goes? If you run into the same issue again, let us know, and we'll do some more testing to see what's going on.
