How to let form filler to select only months

I have a Jotform (New Client Checklist) we use to set up new clients. I would like to include a date picker that only allows/requires a month to be chosen. That is, I do not want them to select a date or a year, as with a traditional date picker, just a month.

Is that possible?

Thanks for reaching out to Jotform Support. You can use DatePicker Year Month widget on your form to select only month and year. Let me walk you through it:

1. Open your form in Form Builder, click on the Add Form Element menu on the left side of the screen.

2. Under Widget tab, search DatePicker Year Month and drag and drop it on your form.
