I forgot the username and email address associated with this form

Thanks for reaching out to Jotform Support. I've checked the form that you've shared with us and saw that it belongs to another account with the email address, which is a***n@l*****e.pe. If you recognize this email address, you can use it to log in to that account. But if you've forgotten the password of that account, you can simply reset your Jotform password. Let me show you how to do it:

1. In Jotform Main Page, click on Login on the upper-right part of the screen.

2. Click on Forgot Password.

3. Type your preferred email address on Enter Your Username Or Email field.
4. Click on Send Reset Instructions.

5. In your email service provider, click on the email notification labeled Jotform Lost Password.

6. Click on Reset Password.

7. Type your preferred password on New Password field.
8. Type again your preferred password on Verify Password field.
9. Click on Create A New Password.

After that, you can use your new password to log into your Jotform account.
