I contacted Support and they informed me that the account would be unlocked, if it was not released, I would need to contact support again.

Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named Cadastro para nova Senha Condomínio Cerro Mirador that’s breaking our Terms of Use by collecting login credentials.
I've deleted the offending fields, updated your form name and reactivated your account.
