Personalise a standard form

    I have a standard form which I send to clients where they respond to a piece of text with various answers.
    The problem I have is that each form will have a different piece of text dependent on the client situation.

    Is there a way to personalise each form in terms of this particular piece of text but retaining the input to be answered the same for each client.

Rest assured, we’ll find a solution for this. Can you share with us a screen recording of what’s happening so we can try to replicate it on our end? You can share the video recording on any public sharing platform like Google Drive or Dropbox, or you can simply use Loom to record the video and share the video link with us.

    Once we hear back from you, we'll be able to move forward with a solution.

    sorry - it is a conceptual question as I have no form at this stage. Part of our business process is to agree on a scope of service for our client. This will vary from client to client. It could be to 'review clients personal situation and advise on insurance solutions to protect etc etc etc' or it could be 'invest $50,000 in a low risk fund with a tieline of 10 + years'

    I want a form which will reflect the 'scope of service' pertaining to that client then add a number of questions to follow - example 'I agree with the scope of service' or I agree with the scope of service but wish further advice on the following'

    The form will also have a couple of terms of business they will agree or not agree to.

    The issue is of course how I tackle the scope of service and include it in the form when each scope of service I include will be different to others - not standard wording.

    I could put the scope of service wording in the email in which I send the link to the client which could work for me however my preference would be to add it to the actual form if there is a way of doing that easily. If there is no way then I am content to fund an alternative for the scope of service

    Hi Tony,

    Yes, it is possible to create a custom form based on the users selection. However, you need to set up all questions to each selected answer. Then you need to set up show/hide field conditions for the questions based on their previous answer. In this way you can personalize the questions based on the needs of your form user. Let me show you how:

    1. In Form Builder, click on the Add Form Element menu on the left side of the page.
    2. Under the Basic tab, scroll down to Single Choice, and drag and drop a Single Choice to your form.
    3. Assuming you have two possible choices on the Single Choice Element, you need to add 2 sets of future question fields that are hidden, which will only be shown once an answer is selected.
    4. Repeat the same steps for Question 2 and create a set for your question 3.

    Setting up the conditions.

    1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.

    2. Click on Conditions in the menu on the left side of the page. Then click on Add Condition.

    3. Select the Show/Hide Field condition and set up your conditions based on the screenshot below:

    4. Make sure each answer for all your questions has a condition set up, for the flow to continue.

    You also need to pre-plot your additional questions and use the same principle above, the only difference is you will fill up the one field to show the additional questions that is fit for the customer's profile. Let me show the results on the screencast below:

    You can use the Approval workflow so that you can set up the editing of the form once the user submits the form. And once your done editing his form, you can send it back to the form user to complete. You can check this guide on how the approval workflow works.
