Approval Workflow requesting approvals on already approved form

Management reports the below workflow approval does not appear to be working. Users stated that an already approved request had re-requested approval. Please advise as we have stopped using form.

Thanks for reaching out to Jotform Support. I checked Gary's email address, and no new approval email had been issued to him. I also noticed that the two submissions were incomplete in the first place. This indicates Gary or the recipients did not approve or deny the two pending approvals.
