Como posso criar um formulário/site ou app para a COPA

Como eu consigo criar um site/formulário ou app para palpites diário. Nesse app/formulário/site, os clientes iriam no Totem para registar seu palpite, com nome, numero de telefone e seu palpite do dia dos jogos que escolhermos.

No final do dia, exportamos um relatório (EXCEL) com todos que registraram e com esse relatório conseguimos identificar os ganhadores do dia.
