Falha no preenchimento do formulário

Boa tarde! Os meus formulários não estão funcionando corretamente. Quando o usuário abre o link para preenchimento demora muito para abrir, e a medida que o preenchimento ocorre, o formulário da erro (principalmente nas fotos)

Now, coming back to your question, is this happening on a specific one of your forms? Or is it on multiple forms? If you could actually provide me with the Form IDs/URLs, I can clone them and investigate further.

I was unable to replicate your issue on my end after cloning your form. It seems to function just fine with no issues.

Would you be able to capture a screenshot of the errors you have seen happen to your users?
