Problemas com envio de e-mail.

Estou com um problema para alterar o assunto do e-mail do meu formulário para ser o nome do assinante. Na configuração do e-mail na parte de assunto eu selecionei a seguinte mensagem:

"TERMO VITRECTOMIA: (NOME COMPLETO DO PACIENTE:) (CPF DO PACIENTE)"
mas a mensagem que está chegando é : Your TERMO VVPP was signed successfully!




Porem no e-mail não chega nem o assunto nem o corpo do e-mail.

Vou mandar como fica no e-mail apenas com essa mensagem:

Now, getting back to your question. Unfortunately, changing the Email Notification's subject line will not affect the Jotform Sign notification's subject. Jotform Sign's email notification is sent to the form owner after the user signed your form. And unfortunately, this subject line that tells you that Your TERMO VVP was signed successfully cannot be changed. While the feature you’re looking for isn’t currently available at Jotform right now, rest assured that we're looking to add more features to Jotform. When or if the features are developed depends on our workload, how viable they are, and how many other users request for the same feature.
