Meu formulario esta com problemas de PII

Fiz um formulário para dar um curso de Segurança do Trabalho mas o mesmo esta indisponível pois o sistema alega que estou utilizando dados PII.

Acontece que este certificado só é valido se tiver o nome, RG e CPF do aluno.

Como resolvo isso??
