Vi que na configuração do formulário, existe a opção do Campo Exclusivo, existe como incluir mais de um campo?

Now, about your question, this is possible by adding the values of several fields into one unique field using conditional logic, and then configuring this field in the Unique Field option in the Form Settings. Let me show you how to do it:

1. On Form builder, add a short text field to the form, and then go to the Settings tab.
2. Click on Conditions on the left side, and create a condition to update/calculate a field, including the fields from which you are bringing data. In this example, there will be email and ID number fields.
