modulo per appuntamenti

salve,

ho un problema sul mio sito dopo aver incorporato il codice sorgente del modulo prenotazione consulenza, quando provo a compilare il modulo e clicco su prenota consulenza mi dà errore che ci sono problemi nei campi modulo, ma non è evidenziato nessun campo, a volte mi dà errore sul sul campo appuntamenti, ma non riesco a capire perchè, la prenotazione è fatta correttamente.
