I checked your form and see numerous CSS Codes that were added to change the positioning of your fields.

I can't review them line by line, but if you're not the one who did it, I suggest contacting the person to explain things to you. Also, you can always reach out to us in case you're having trouble embedding forms to your website.

I reviewed your previous tickets and none of them is to center your form fields. You may want to check out with your Graphic Designer as well, perhaps something was done in your website to force your embedded form to display in center.
