kenapa akun saya di suspend ya? dimohon bantu dibuka kembali akun saya sepertinya saya tidak melanggar kebijakan jotform.

Now, coming back to your question, your account was suspended because you’ve got a form named KUIS TEBAK SKOR NAGALIGA that was tagged as spam/phishing. We understand that this might not be the case with your form, but we had to act according to the general rule to be in compliance with our Terms of Use. I did check your form, and it looks like you are collecting user ID which is not allowed.
If you agree to follow our Terms of Use and let us delete the offending fields mentioned above, we can go ahead and lift the suspension on your account.
Once we hear back from you, we'll be able to move forward with this.
