Resident Assistant Reference Form
A Resident Assistant Reference Form is a document created by a resident assistant to provide information about their assigned residents. It is a reference form for building staff, and the person being referenced. It is completed by the RA, and the building staff (Director, Assistant Director, Hall Director, etc.) to provide information on the residents living in their building. The form is used by the building staff to provide information to residents who are new or returning, leasing staff, or others in the building. The form provides information on the resident’s relationship with the building staff, how they can be contacted, and what their role on the hall is.
Buildings that have a lot of turnover may also have this form to quickly gather information from new residents. Information may be updated in the form over the course of a resident's stay. This form may be used in addition to larger, Resident Questionnaires, and Collaboration Forms which collect more detailed information.
