The medication and vaccine section is using the Configurable List widget that allows the respondent to add more similar fields based on the number of data they want to share. This form template is also using the Terms and Condition widget wherein this required field should be checked in order to complete the submission. This template also uses the Signature tool where the pet owner can sign after acknowledging this waiver and consent form.
