Field Change Order Form
A Field Change Order Form is a document used by the contractor to identify the areas that can be modified or upgrade to improve the project. A good change order form should thoroughly explain why the following modifications need to be done in the project. This should also provide a financial analysis so that the client will be able to understand and approve the changes.
This Field Change Order Form contains form fields about the client, information about the project, financial budget, e-signature, and reasons for the modifications. This form template uses the Input Table tool in order to display the modifications in a table format. This table is asking for the description of the changes, reason, priority, and the type of change, and amount. This template is also using the Unique ID widget in order to assign a change order number for each submission. Using the Form Calculation widget, this template automatically calculates the costs of each change and capture the total amount. This form template is also using the E-signature widget to obtain a digital signature from the approver/client and the contractor.
